California Healthy Kids Survey

Survey Information

The California Healthy Kids Survey (CHKS) is the largest statewide survey of resiliency, protective factors, and risk behaviors in the nation. It is a powerful tool for use in Grades 6-12 that can help schools and districts accurately identify areas of student and school strengths and weaknesses, and address related needs. It provides a comprehensive, data-driven, decision-making process to guide efforts to improve school climate, learning supports, and engagement, as well as identify and increase the quality of health, prevention, and youth development programs.

Across California, the CHKS has led to a better understanding of the relationship between students' health behaviors and academic performance, and is frequently cited by state policymakers and the media as a critical component of school improvement efforts to help guide the development of more effective health, prevention, and youth development programs.

TTUSD administers this survey to 6th, 7th, 9th, & 11th graders on a bi-annual basis. All parents are notified when the survey will be administered. Sixth grade students require active parent consent. Please read the information below for Elementary, Middle School, and High School.

Your 6^th grader is being asked to participate in our District’s California Healthy Kids Survey (CHKS), sponsored by the California Department of Education. This is a very important survey that will help promote better health and well-being among our youth, improve the school learning environment, and combat problems such as drug abuse and violence. This survey is given to all 6th grade students with parent permission.

Participation is voluntary and requires your permission. CLICK on the button below to view Consent Form and the Student Survey.

Se le pide a su hijo de sexto grado que sea parte de nuestra Encuesta California Healthy Kids patrocinada por el Departamento de Educación de California (CDE). Esta es una encuesta de suma importancia que ayudará a promover una mejor salud entre nuestros jóvenes, mejorará el ambiente de aprendizaje en la escuela, y combatirá problemas como el abuso de drogas y la violencia. Esta encuesta se le da a todos los estudiantes de 6° grado con el permiso de los padres.

If your child is in 7th, 9th or 11th grade or attends Sierra High School, they are being asked to be a part of TTUSD’s California Healthy Kids Survey (CHKS) sponsored by the California Department of Education. This important survey will help promote better health and well-being among our youth, improve the school learning environment, and combat problems such as drug abuse and violence. Your child does not have to take the survey.

If you do NOT want your child to participate in the California Healthy Kids Survey, please click here and complete the form by May 23, 2025.

If you ALLOW your child to participate in the California Healthy Kids Survey, no further action is necessary.

7th, 9th & 11th GRADE STUDENTS

Si su hijo está en el grado 7, 9 u 11 o asiste a la escuela secundaria Sierra, se le pedirá que participe de la encuesta California Healthy Kids Survey (CHKS) de nuestra escuela patrocinada por el Departamento de Educación de California (CDE). Es una encuesta muy importante que ayudará a promover una mejor calidad de salud y bienestar de nuestros jóvenes, mejorará el ambiente de aprendizaje en la escuela y combatirá problemas como el abuso de drogas y la violencia. Su hijo(a) no tiene obligación de participar en la encuesta.
